What New Patients Need to Know

We’re so excited to meet you in person! For your convenience, you can download our new patient paperwork and get a jump start before your first appointment. If you can’t fill out the forms before your appointment, be sure to arrive 10-15 minutes early so you can complete them in our office. We will have blank copies for you once you arrive if you need them.

Kindly leave all medical history and signature lines blank, so we can fill them in together when we see you!


How to Fill Out Our New Patient Forms

By downloading and printing the forms, you can streamline our new patient process. Either fill out the packet by hand or use Acrobat Reader to enter all necessary information before printing. You can also save paper and email us your completed packet if you’ve filled it out digitally.

Please wait to sign these forms until you arrive in our office. You should only complete this form after it has been downloaded to your computer. To do that, “right click” on the button above and SAVE to your computer in order for it to download properly. Call us if you have any concerns. We will gladly email the New Patient Packet to you.

Parking Tips for New Patients

When you come to our office in West Hollywood, we recommend using one of the paid parking lots in the area. Your most convenient option is the lot right outside of our building, with a daily maximum of $16.00. It’s only steps from our front door. The other lot is just one building to the east of us on the north side of Wilshire. You will see an ATHENA parking sign. This lot is only a slight walk. Athena Parking’s daily maximum is $14.00.

There are one-hour meters around our neighborhood, but since most appointments are scheduled for at least an hour, we strongly advise against using metered parking. Metered parking is very limited due to the new transit lanes on Wilshire. To avoid tickets and towing, we strongly recommend the use of the available paid lots. There is street parking in the neighborhoods surrounding our office as well. Read the parking signs very carefully because parking is an unusual mix of permit and non-permit parking on the same street.
